NTSB recommends changes after Alaska Airlines incident - MSN
The NTSB says a series of failures led to a door plug flying off an Alaska Airlines flight las year. The safety board formally recommended 19 changes to both Boeing and the FAA.
The Alaska Airlines case was not the first time that unauthorized and undocumented work had occurred. "This incident should never have happened. It should have been stopped years ago," said Homendy.
